This is the wording used by Rush University Medical Center regarding
delinquency and fines levied for the medical staff medical records delinquency.

"Any medical staff physician who is suspended due to delinquent medical
records four (4) times in any twelve (12) consecutive months will be fined
one thousand ($1000) dollars.  Once a physician is suspended and fined, the suspension
will not be removed until 1): all outstanding reports are dictated and signed, AND 2) the
$1000 fine is paid in full."
